Sports Physicals

Each year, prior to participation in any try-out, practice, athletic class, open gym, open weight room, athletic competition or traveling with a KISD athletic team for any purpose, students must submit athletic participation forms.

This is a requirement of all local school districts, and can help identify any reasons that might prevent athletes from participating in their activities of choice. It also gives an opportunity to address any issue that comes to light, and improve existing condition, so athletes can enter into the year in peak performance.

The great news is that you can now do this online, so the process is even more simple and convenient. You can do that here.

The annual physical and medical history form is the only one that can’t be completed online, although you can print one of these to take to your physician while completing your other forms.
